PRE-REGISTRATION FORM CLOSED SUPPORT GROUPS

Please note that the closed support groups are offered in Fall (middle of September) and in Winter (middle of January) The closed support groups are offered online or in the Greater Montreal Area, including Laval and the South Shore. Certain closed support groups (i.e. Level 2, Sexuality and Nutrition) are generally offered in the Winter. With this registration form, you will be added to our call-back list. This registration form does not guarantee you a place in the next group. You will receive an e-mail from the clinic coordinator 3 weeks before the first meeting of the closed support group. The e-mail will give you more information about schedules and locations. Please note that you will receive this email only if the group desired is offered. When you receive the e-mail, you will have to contact ANEB, either by replying directly to the clinical coordinator by e-mail. You will then be invited to choose the location and desired schedule. Please note that your answer to this email will be the last step before being officially registered in a closed support group. If we do not receive an answer from you, you will not have a place in the group. The placement of people into closed support groups will be done on a First come, first served basis. If you answer the e-mail and all the closed groups are full, your information will be kept on our waiting list. You will then be contacted again during the next group registration period. Please note that your information will be kept for a maximum of 3 years, after which your name will be removed and you will have to fill out another registration form.
